hiring a Nurse Practitioner for our PACE Program This rewarding

and adults with physical and intellectual disabilities continue to
live as independently as possible in the community. This is a
part-time opportunity at our Kenosha facility. $8,000 sign on bonus
Nurse Practitioner Job Responsibilities:--- Documents and updates
health history and findings, physical exam, functional status
evaluations, diagnostic test and lab evaluations, mental health
status and function evaluations, preventative care, and health
reminders.--- Within their official scope and practice, delegates
appropriate tasks to interdisciplinary team staff, and provides
leadership and guidance. Acts as a resource to staff to ensure
appropriate care and follow through and provides care planning
assistance.--- Educates members and families regarding health
maintenance and acute and chronic illnesses.--- Travel, Regular
local travel, requiring use of personal vehicle, valid driver's
license, and proof of required insurance coverage.Nurse
Practitioner Job Requirements:--- Education: Masters degree in
Nursing required.--- Licensure: Wisconsin Registered Nurse/RN
license required. Family, Geriatric, or Adult NP certification is
required. DEA registration required. Advance Nurse Prescriber
required. NPI and CPR & AED required.--- Experience: minimum of 1
year recent clinical nursing experience with an at risk or elderly
population.Registered Nurse/RN Job Schedule:--- Monday to Friday,
