Job Description:

This position is responsible for providing case management services to children with severe emotional disabilities (SED) and their families who are at risk of psychiatric inpatient hospitalization or other out-of-home placements. Case management services will incorporate the wraparound model of service delivery in which supports in the home, school, community, and other areas of the child's life will be encouraged to take a team approach to assist the child and the family in meeting their needs. Case management approach will include a strength-based and person-centered approach to service delivery.Salary: $19.23 an hour.$1,000 sign-on bonus.$ 1,000 raise every 6 months, for the first 2 years of employment.Key Responsibilities:Interview and evaluate clients and families to complete the intake and assessment process (CANS and Functional Screen).Complete intense, Medical Assistance (MA) compliant Strengths and Needs assessment of child and family.Assess client situations, capabilities, and needs to determine services required to meet the treatment goals of the client and their family.Develop, implement, evaluate, and revise outcome oriented and strength based treatment plan (ISP) as changes occur.Develop a family support team comprised of formal and informal supports.Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.Develop a crisis/safety plan for the client and family.Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.Attend client/family court proceedings, IEP meetings, psychiatry appointments, or other appointments that may require family support or advocacy.Incorporate knowledge and philosophy of Coordinated Services Team (CST) into daily work with clients.Become certified to conduct initial and annual screens to determine eligibility for Comprehensive Community Services, Children's Long Term Support Waiver, and Children's Community Options Program funding benefits for all program clients.Utilize all the available MA funding benefits such as Targeted Case Management, Children's Long Term Support Waiver, Comprehensive Community Services, and Children's Community Options Program while complying with their specific regulations.Utilize advanced knowledge of services and resources in the community and be willing to research new and unknown resources to assist the client and family in meeting their goals.Work in collaboration with other providers, school personnel, mental health professionals, Division of Children and Family Services workers, and court officials.Advocate for and teach advocacy skills to clients and families while attending team meetings, school/IEP meetings, Division of Children and Family Services staffings, and court hearings.Formulate daily contacts into units entered into various databases.Meet all deadlines of documentation including entering progress notes, reviewing provider progress notes, assessments, individual service plans, safety plans, service authorizations, quarterly reports, annual re-certifications, and discharge summaries; this includes allowing time for Supervisor and Clinical Services Director to review these documents.Administer surveys to families to collect data on family satisfaction.Meet requirements of state reporting and outcomes reporting through the use of Program Participation Survey (PPS) and the Families First Sharepoint Survey.Participate in weekly program staffing and present assigned cases.Receive clinical supervision and implement recommendations into a treatment plan.Report client behavior to courts and Kenosha County Department of Human Services and make appropriate recommendations.Attend trainings to keep abreast of new information related to the mental health field.Ability to operate a motor vehicle safely and obey all traffic laws.Safely transport clients as needed.Other duties as assigned..

Must have reliable transportation on a daily basis, a valid driver's license and automobile insurance.Education and Training:Bachelor's Degree in Social Work, Psychology, Criminal Justice, or other related fields.The position must keep current on information pertaining to client/family needs through seminars, trainings, media, text, and research.Experience:One year's experience working with the severely emotionally disabled population. (Preferred)Knowledge, Skills, and Abilities:Ability to read, speak, and write clearly in English.Ability to exercise good judgment and discretion in establishing a daily work schedule.Must be able to work cooperatively and collaboratively with the Department of Human Services.Must establish and maintain productive and professional relationships with clients, colleagues, the school district, and other community agencies.Must maintain client confidentiality and follow HIPAA requirements.Ability to prioritize and make appropriate decisions.Ability to work independently and effectively manage time.Ability to work in a team environment.Must possess strong oral and written communication skills and be able to present case information in staffings with the Department of Human Services.Ability to use various electronic databases with updated client information and services.Ability to provide culturally, ethnically and gender sensitive services.Ability to follow directions and implement recommendations.Possess creativity in identifying problems and solutions.Ability to self motivate and remain on task.Knowledge and compliance with policies and procedures as detailed in the personnel policy and procedure manual.Knowledge of diagnoses listed in the DSM V.Knowledge of community resources.Equal Employment Opportunity/M/F/disability/protected veteran statusPI160671410
