Work Schedule: /n 100%, day shift working Monday – Friday, 8:00 am
– 5:00 pm. will work at University Hospital and Eastpark Medical
Center in Madison. /n Be part of something remarkable /n As an
Advanced Practice Provider, you'll play an integral role in our
ambulatory care team by providing high-quality, cost-effective care
to patients. /n We are seeking a Nurse Practitioner or Physician
Assistant to: /n /n - Work with a multi-disciplinary care team in
Breast Medical Oncology. /n - Conduct comprehensive or episodic
health history and physical assessment of Breast Cancer patients
with acute, chronic, and/or potential health problems. /n - Oversee
treatment infusions and supportive care needs. /n - Manage and
modify treatment plans of patients as needed. /n - Provide
ambulatory urgent call coverage for Breast Cancer patients, as well
as other oncology tumor types as needed. /n - Facilitate referrals,
procedures, and inform ambulatory care coordination during
discharge of hospitalized Breast Cancer patients /n /n At UW

Practitioner: Master’s or Doctorate’s degree from accredited
Nursing Program. /n If Physician Assistant: Bachelor’s or Master’s
degree from accredited PA Program. /n Licenses and Certifications:
/n Minimum - /n If Nurse Practitioner: Active RN and APNP license
in State of Wisconsin AND active board certification as a NP in
Adult; Adult/Gerontology Primary Care; Family; Gerontology. /n If
Physician Assistant: Active PA license in State of Wisconsin AND
active PA board certification. /n All Nurse Practitioner and
Physician Assistant applicants must have: Active DEA license or
ability to apply for such license prior to or upon hire AND CPR
certification and advanced life support certifications if relevant
