Job Description UW Health is seeking a Nurse Practitioner Family Practice for a job in COTTAGE GROVE, Wisconsin. Job Description & Requirements - Specialty: Family Practice - Discipline: Nurse Practitioner - Duration: Ongoing - Employment Type: Staff 100%, days. Monday through Friday. Hours may vary based on the needs of the department. You will work at the Cottage Grove Clinic. /n Be part of something remarkable /n As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients. /n We are seeking a Nurse Practitioner OR Physician Assistant to: /n /n - Perform annual physical exams and manage the health maintenance needs of our family medicine population. /n - Care for the acute and chronic needs of patients from newborn to elderly. /n - Perform telemedicine (video and telephone) visits. /n - Perform routine clinical procedures independently including wart removal, pap smears, IUD removal, IUD/Nexplanon placement and skin lesion removals. /n /n Education: If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program. If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program /n Licenses and Certifications: /n Minimum – State of Wisconsin NP or PA licensure /n If Nurse Practitioner*: /n /n - Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND /n - Active board certification as a NP in area of clinical practice.
